Interested in scheduling an appointment online? If so, please send us an e-mail at dr.bend@ymail.com typing NEW PATIENT APPOINTMENT in the subject line, then type your name, phone number and 2 dates that you are available for an appointment in the body of the e-mail message.
Once we receive your e-mail, our staff will contact you to confirm your appointment for one of the dates you specified.
Next, download our New Patient Application form, fill it out completely on both sides and bring it with you to your scheduled appointment. We look forward to meeting you at your scheduled appointment time!
